Confirm the actual setting and coverage dates with Medicare or a qualified counselor before tying a home sale or next lease to a plan assumption.

Use the institution rule for the right setting

Medicare lists an institutional Special Enrollment Period for someone who lives in, or recently moved out of, an institution such as a nursing home or rehabilitation hospital. During the stay, the person may be able to join, switch or drop Medicare Advantage or drug coverage. The period continues for two full months after the month the person moves out. Do not apply this rule to every senior apartment or independent-living community by name alone. Ask Medicare whether the actual residence meets the rule.

Do not confuse this with the address-change window

A move outside a plan service area, or to a new address with different plan options, has a separate move-related enrollment rule. Medicare says that window generally starts when the person moves and continues for two full months; notice before the move can start it earlier. The institution rule has different timing. An Oklahoma household may have both a facility discharge and a new home address, so write both events and dates on one sheet and confirm which election opportunity applies. A real-estate closing date is not necessarily the Medicare move date.

Before leaving a facility for an OKC-area home, obtain the exact new ZIP code, expected discharge date, medications, preferred physicians and any ongoing therapy or equipment needs. Compare plan service areas, networks, pharmacies and drug coverage using current plan information. A plan available at an old address may differ at the new one.
